In Person
Requests for birth, death and marriage records can be submitted in person during our normal business hours. All persons requesting records must complete the application in full, be a qualified applicant, have a valid form of identification and proper payment at the time of request.
By Mail
Requests for birth, death and marriage records can be submitted by mail. All persons requesting records must be a qualified applicant, complete and transmit the completed application, notarized Affidavit of Personal Knowledge, a copy of his or her valid proof of identification and proper payment.
